Acknowledgements

I recognize the program(s) may include strenuous physical activity including, but not limited to, muscle strength and endurance training, cardiovascular conditioning and training, and other various fitness activities. I herby affirm that I am in good physical condition and do not suffer from any known disability or condition which would prevent or limit my participation in this/these program(s).

I, {name}, also recognize that despite all reasonable precautions implemented for safety, potentially serious injuries including paralysis or death may occur in any activity involving contact or motion.

I fully understand that I may injure myself as a result of my enrollment and subsequent participation in aforementioned program(s).

Releases and Permissions

I knowingly and willingly assume all such risks and therefore consent to participating in Jiu-Jitsu Unlimited LLC programs. I acknowledge my enrollment and subsequent participation is purely voluntary and in no way mandated by Jiu-Jitsu Unlimited LLC.

Consequently, I, {name}, herby for myself, heirs, executors, and administrators, waive and release any and all rights and claims for damages against Jiu-Jitsu Unlimited LLC, it’s members, operators, coaches, affiliates, and subsidiaries from personal injury or accident of any nature suffered by my person by reason of my voluntary participation or enrollment in subsequent program(s).

Media Release

I, {name}, fully authorize and grant Jiu-Jitsu Unlimited LLC and it’s authorized representatives the right to print, photograph, record, and edit as desired the biographical information, names, images, likeness and/or voice of the above named pupil. I understand that use of such recordings/images will be without any compensation to members.
